Tree Fern Oyster Mushroom (Pleurotus cyatheae)  

This species, described in 1939, is known only from two localities on the island of Chichijima in the Ogasawara Islands, Japan, where it apparently grew on the stems of tree ferns.

The species is considered extinct. [1]
